A Message from Our Leadership
This past year was one of growth and focused action at Vibrant Community Health. Guided by our 2025–2028 Strategic Plan: Equity, Access, Impact, we expanded access to care, strengthened our services and partnerships, and continued responding to the needs of people who experience barriers to care.
Primary care expansion was a major priority. Through provincial investment in interprofessional primary care and our work with the North Toronto Ontario Health Team, we increased our capacity to connect more people without a regular primary care provider to ongoing care. We also strengthened relationships across North Toronto, helping clients access primary care, allied health, wellness programs, and community supports.
We also invested in the systems that support quality service delivery. In Attendant Care, we advanced GoldCare implementation to strengthen client information management and care coordination.
Serving our attendant care clients, Vibrant invested in a this new database for collecting and storing information, better serving our clients at three locations. Vibrant leadership also took a Board of Directors role with the Ontario Association of Independent Living Service Providers (OAILSP).
The year marked Vibrant’s first full year following integration with Sunnybrook Health Sciences Centre. This partnership is creating stronger connections between hospital, primary, community, and attendant care while preserving Vibrant’s community-based identity and commitment to equity-deserving populations.
We are grateful to our staff, volunteers, community advisors, partners, and funders. Most importantly, we thank our clients and community members. Your trust, feedback, and participation continue to shape Vibrant.
As demand for accessible care and community supports continues to grow, we enter the year ahead with a stronger foundation, valued partnerships, and a clear direction. Together, we will continue working toward an engaged, healthy, and inclusive community where people can access the care and support they need to live independently and well.

Vibrant offers a range of health, wellness, and community-based services designed to support your overall well-being.
Attendant Care: Vibrant also offers assisted living to clients with physical disabilities at 3 other separate locations.
Primary Care: Ongoing clinical care delivered by doctors and nurse practitioners, focused on prevention, treatment, and long-term health management.
Allied Health Care: Services include counselling, diabetes education, physiotherapy, occupational therapy, case management, chiropody, nutritional supports.
Community Health & Wellness Programs: Group and community-based programs that support physical, mental, and social well-being. We also support clients to navigate appropriate supports.
All programs and services vary based on availability and eligibility.
Who We Serve
Vibrant serves individuals and families in North Toronto who benefit from coordinated, accessible, and community-based health care. We also accept clients living with physical disabilities from anywhere in the GTA.
Our clients include people of all ages, including those who are living with complex health conditions, physical disabilities, and/or social and economic barriers that affect their health and well-being.

Our Journey: Building Capacity and Expanding Access
This year, we made important progress to strengthen our foundation and create more opportunities for people in our community to get the care they need, now and in the future.
April 2025 — A New Chapter
Vibrant began a new chapter through its integration with Sunnybrook Health Sciences Centre. Work continued throughout the year to align administrative systems, strengthen governance and financial processes, and support better planning across the organization. Clients continued receiving services while this important behind-the-scenes work took place.
2025/26: Improving Client Pathways
Vibrant secured $3.2 million in new base funding through Ontario’s Interprofessional Primary Care Team initiative. This funding supported the recruitment and onboarding of additional primary care providers, allied health professionals, medical secretaries, and program leaders.
2025/26: Strengthening Our Spaces and Systems
Vibrant invested in technology, information systems, security, and physical infrastructure. Improvements included stronger internet service, email and Teams migration, multi-factor authentication, enhanced data reporting, security upgrades, and facility improvements.
What Comes Next
Vibrant will build on this foundation by continuing to improve intake, triage, referral, and care coordination. The organization also plans to increase primary care attachment, reduce wait times, expand wellness programs, and improve access for equity-deserving communities.
Clients can expect clearer service information and policies, continued improvements to technology and phone systems, and additional clinical space through the lower-level renovation. These improvements will be introduced gradually, based on available staffing, funding, and service capacity
