Our leadership team led by a dedicated group of healthcare professionals and community advocates, our leadership team brings diverse expertise and a shared passion for improving health outcomes. The Board of Directors provides strategic guidance, ensuring our services align with our mission. Our Community Council, comprised of clients, ensures that community voices are heard in our decision-making processes. Supported by our senior staff, comprised of experienced healthcare professionals, we strive to innovate and expand our services to meet the evolving needs of our community.

Leadership

  • Lavinder has over 20 years of experience in the healthcare sector, beginning her career as a Personal Support Worker and gaining a deep understanding of care from the front line. Throughout her career, she has grown into leadership and management roles, where her focus has been on empowering teams and ensuring individuals receive the highest quality of care.

    She is a passionate advocate for the independent living philosophy and believes in promoting dignity, respect, and choice for every person. With her empathetic and relationship-driven leadership style, Lavinder is committed to creating inclusive environments where both staff and clients feel valued, supported, and truly worthy.

  • Hyder has 25 years of experience in the non-profit sector, including healthcare. Having worked nationally and internationally, he brings expertise in project and program management, risk management, policy and strategy development, monitoring and evaluation.  Hyder has competencies in client relationship management, as well as stakeholder and government relations.

  • Lori is a Registered Physiotherapist with over 20 years of progressive leadership experience in hospital, home and community care, and health system planning sectors. She has led strategic planning, clinical operations, and innovation initiatives at both system-wide and local levels. Lori has a record of propelling transformative change and is a strong advocate for patient-centred and community-based care.

    Lori serves as the VP, Community Integration, Partnerships & Outpatient Care for Sunnybrook Health Sciences Centre

  • Cameron has 19 years of experience in the non-profit healthcare sector. Having worked with non-profits focused on grassroots-based international development, HIV, Parkinson’s disease, and end-of-life rights, he brings exceptional experience in strategy development and client relations.

    Cameron has a Masters degree in International Studies from Stellenbosch University (South Africa).

    He is passionate about Vibrant’s mandate as a Community Health Centre and Attendant Care Service Provider focused on providing holistic services to clients with barriers to care.

  • Janice brings extensive experience in public health and community leadership to her role at Vibrant Community Health. Having served as a public health educator for a state health department, she led community outreach and education initiatives to strengthen local engagement and public health preparedness. Before joining Vibrant, she also directed a multidisciplinary volunteer team through the Medical Reserve Corps program. Passionate about collaboration and equity, Janice is committed to fostering connection, compassion, and inclusion, especially amongst those facing barriers.

  • My Dang is a seasoned Decision Support and Information Management Lead with over 25 years of experience in the healthcare sector. She is highly proficient in a variety of databases and data analytics, specializing in data transformation into actionable insights that drive strategic decision-making and enhance performance management. With a strong background in digital and virtual healthcare technologies, My possesses a deep understanding of electronic health records (EHR) and the importance of systems integration to improve both patient and provider experiences. She is a dedicated leader who is passionate about leveraging technology to advance and optimize healthcare outcomes and service delivery in the healthcare industry.

  • Constantine has over thirty years experience in the non-profit sector, including community support and health care. Areas of involvement include program development, volunteer management, client advocacy, fundraising and grant writing, and resource counselling, and client advocacy and support systems navigation.

  • Kiros has over 30 years of Community Health Services experience responsible for the management of site operations and the provision of attendant care services including planning, directing, monitoring and training of Personal Support Workers (PSWs) and the management of service delivery issues.

  • Darienne has over 10 years of experience working in Client Centred & Customer Service roles, across various disciplines, in addition to their long history of operating in positions of Leadership. When leading a team, they aim to lead through pro-active responses, open communication and transparency; All while striving for peak efficiency, with the primary goal being elevation of client experiences. While Vibrant Community Health is the first non-profit organization Darienne has been a part of, they are eager to build and grow within the organization and to demonstrate the best possible client care for our valued clientele.

  • Dennis has over a decade of experience in the healthcare sector, combining strong clinical expertise with proven leadership in program management. He began his career as a Physician Assistant, delivering direct patient care, before transitioning into health program management.

    Throughout his career, Dennis has held key roles including Clinical Coordinator and Program Manager, where he supervised interdisciplinary health teams and led the implementation of equity-focused initiatives across underserved districts in Africa. His work reflects a deep commitment to improving healthcare access, strengthening health systems, and ensuring fair and inclusive care for all communities.

  • Allison has over 6 years of experience across multiple sectors of community services, including community healthcare, harm reduction, housing, and shelters. In her current role, Allison oversees the primary healthcare and allied health teams, bringing with her several years of management experience in a community service setting. She is well-versed in system navigation across Toronto, and is a strong advocate for underserved populations. Allison strives to provide access to services that meet people where they are at in life.

  • Michele Heath has worked in management and leadership positions in CHCs and community-based organizations for over 30 years.  She brings extensive experience in program planning, development and evaluation as well as community and sector engagement and partnerships.

Vibrant Community Health Advisory Committee

The Vibrant Community Health Advisory Committee (VCHAC) serves as an advisory body to support the organization’s mission of providing equitable, community-based care. The committee is composed of community members, clients, and staff who offer insight and feedback on Vibrant’s programs, services, and strategic initiatives. VCHAC ensures that the perspectives and experiences of the community are reflected in organizational planning and decision-making. Guided by the principles of inclusion, equity, and the Independent Living Philosophy, the committee plays an essential role in advancing Vibrant’s commitment to accessible, person-centred care.

  • Laura Mattila is Executive Director of the Ontario Medical Association’s (OMA) Physician Health Program. Prior to joining the OMA, Laura held healthcare leadership positions at the Centre for Addiction and Mental Health and SickKids Hospital in Toronto as well as Sault Area Hospital in Sault Ste. Marie, Ontario. Laura has held volunteer executive board of director positions in the non-profit sector in the areas of community health & supportive housing, mental health, and women’s employment. Laura completed a Master of Arts degree from the University of Toronto with a specialization in Sociology of Health & Mental Health, and a Master of Business Administration from Carleton University with a specialization in Management & Change.
