Vibrant Community Health (formerly Vibrant Healthcare Alliance) provides integrated, accessible, and equitable community-based healthcare, attendant care, and wellness programs.  As both a Community Health Centre and Attendant Care Service Provider, we actively engage and prioritize outreach and education, fostering a culture of healthy communities. Vibrant believes that true health extends beyond the specific services we offer and into the everyday lives of our clients.  

Our Vision is for engaged, healthy and inclusive community.

Who we Serve

At Vibrant, our focus is on breaking down barriers to care, ensuring that everyone, regardless of their circumstances, has access to the support they need to lead healthy and independent lives. Guided by our values, we strive to improve health outcomes by increasing access to healthcare and attendant care while advocating for health equity.

Vibrant is dedicated to serving a diverse range of individuals within the City of Toronto. Our primary commitment is to those who encounter barriers to receiving care. The populations we focus on include people with physical disabilities and seniors with complex health conditions. As we work with an equitable lens, we want to find ways for clients to have ease of access to the types of services they need.  

Our Affiliation with Sunnybrook Health Sciences Centre

In April 2025, Vibrant Community Health completed a governance restructuring integration with Sunnybrook Health Sciences Centre (“Sunnybrook”).  The integration was undertaken to  support and enhance Vibrant’s healthcare, attendant care, and wellness programs.

 As part of the integration, Sunnybrook will provide some administrative services to Vibrant which will enable  Vibrant resources to focus on enhancing and growing the services that we are able to provide as a Community Health Centre and Attendant Care Service provider. Vibrant will continue to operate as a separate organization and will maintain  its operational sites.

Aligning with feedback from Vibrant’s clients, staff, and community members in 2024, key priorities of the governance restructuring include maintaining all existing Vibrant programs, upholding the Independent Living Philosophy, preserving Vibrant’s unique culture, optimizing the Vibrant Health Centre at Yonge & Eglinton, and continuing to serve individuals facing barriers to care, such as those with physical disabilities and seniors with complex health issues.

Benefitting from our history and experience, Vibrant also has a unique opportunity to share insights with Sunnybrook around providing support and care to equity-deserving persons, including people with physical disabilities and seniors.

Our History

Since 2017, Vibrant Community Health (formerly Vibrant Healthcare Alliance) has carried forward the legacy of community support from its parent organizations, the Anne Johnston Health Station and Tobias Attendant Care.

Tobias Attendant Care, established in 1990, offered non-profit housing and personal support services for people with disabilities, promoting independent living and compassionate care across multiple locations.

Founded in 1995 with the advocacy of city councillor Anne Johnston, the Health Station initially focused on serving youth and seniors, expanding over the years to provide comprehensive healthcare services, including providing services to people with physical disabilities.

Both Tobias House and Anne Johnston Health Station brought unique strengths to the table in a 2017 amalgamation. They shared a steadfast focus on providing comprehensive services tailored to our community's diverse needs. Through Vibrant’s healthcare, attendant care and wellness programs, our integrated approach mirrored the holistic care models found in leading institutions, ensuring that every client receives the holistic support necessary for their well-being, independence and health.

Today, Vibrant Community Health builds on this rich legacy, offering integrated services that reflect the diverse needs of our community.
