A Message from Our Leadership

e are proud to present Vibrant Community
Health’s (Vibrant’s) Strategic Plan 2025-2028: Equity, Access, Impact.
This plan serves as a roadmap, designed to guide our organization as we continue to advance community health and well-being.
As Vibrant starts a new chapter, with governance restructuring and affiliation with our long-time partner, Sunnybrook Health Sciences Centre, we are committed to preserving our culture, continuing our focus on clients with barriers to care, while expanding our programs and services. Vibrant’s goal as a Community Health Centre and Attendant Care service provider remains unwavering to provide a client-centred care experience.
Over the past year, Vibrant leadership have engaged in meaningful conversations with clients, staff, partners, and community members to understand the evolving needs of our community and the growing demand for our healthcare, attendant care, and wellness programs. This strategic planning process has reaffirmed our commitment to providing equitable and accessible care while strengthening and expanding our capacity to serve. This plan reflects our shared vision, values, and aspirations for a healthier future.
Ontario is facing challenges with people unattached from primary care, which is felt in the areas where Vibrant serves. The province also has long waitlists for attendant care services that need to be addressed, and there is work to be done to better support individuals with barriers to care. While the government is looking to respond to these issues, Vibrant has a place to advocate for and expand our services to help meet the needs.
Looking ahead, our strategic directions will allow us to provide care for equity-deserving groups who face barriers to care. These directions will serve as our guideposts: Deliver top-quality services for our clients, expand our reach through partnerships, and ensure equitable access and inclusive practices by championing the Independent Living Philosophy. We will advocate for policy change and funding to support connected care for our communities and priority populations.
We extend our gratitude to our dedicated staff, board members, volunteers, and community partners who make this work possible—their commitment and passion are the driving force behind our mission. As we move forward, we invite you to join us in bringing this plan to life.

ibrant is a registered, charitable organization
that provides integrated, accessible, and equitable, community-based healthcare, attendant care, and wellness programs.
With four locations across Toronto, Vibrant serves a community of individuals facing barriers to care through a diverse range of services.
We actively engage in outreach and education, fostering a culture of healthy, connected communities. At Vibrant, we believe that true health extends beyond the specific services we offer and into the everyday lives of our clients.
Our New Name
W
e are proud to share that we have changed our name to Vibrant Community Health (formerly Vibrant Healthcare Alliance).
The decision to change our name reflects the input of our community, clients, and staff. We sought a name that truly represents our collective identity and the values we stand for.

Strategic Plan Overview
In 2024, Vibrant initiated a process to develop a three-year strategic plan to acknowledge the various challenges and opportunities our community identified.
Through client focus groups, board and client workshops, surveys, and partner interviews, we have actively listened to those who rely on our services, those who deliver our services, and those who partner with us to deliver services to the community.
Strategic Directive : Quality Care
Deliver top-quality services for our clients through our healthcare, attendant care, and wellness programs.
We are committed to providing care that is exceptional, responsive, and rooted in best practices. Every client interaction reflects our dedication to inclusion, compassion, and professionalism.
We will strengthen:
• Our internal quality improvement practices and client service protocols.
• The use of client feedback and performance data in guiding service enhancements.
We will build:
• Team capacity to foster self-determination, autonomy, and person-led decision-making, grounded in the values of Independent Living.
• A consistent service model that ensures excellence across all programs and services.
We will explore:
• New models of integrated care and interdisciplinary collaboration.
• Emerging tools and technologies that enhance service quality and efficiency.
Strategic Directive : Client Growth
Expand our reach through partnerships to better meet the needs of our community.
We will increase access to our services to reach more people in our community. By enhancing capacity and streamlining service delivery, we will serve more individuals and families effectively.
We will strengthen:
• Clinic operations and scheduling systems are designed to improve access to care, reduce wait times, and enhance service flow for clients and staff.
• Outreach and intake processes to reduce barriers to care.
• Engagement with sector partners to build stronger referral pathways, shared resources, and work together to better support the people we serve.
We will build:
• Partnerships with other service providers to expand our reach in the community.
• Awareness of our services and programs through community-based promotion and engagement.
We will explore:
• New locations for attendant care services to increase our capacity.
• Innovative staffing models to sustainably increase client volume and create new ways for clients to access care that are flexible, responsive, and person-centred.

Ensure equitable access and inclusive practices by championing the Independent Living Philosophy and prioritizing clients with barriers to care.
We are committed to delivering services that are accessible, inclusive, and responsive to the diverse needs of our community, especially those facing barriers.
We will strengthen:
• Our commitment to the Independent Living Philosophy in all programs and services.
• Anti-oppressive, equity-driven, and culturally safe practices in care.
• Accessibility across our physical spaces, communications, and service delivery
We will build:
• Relationships with underserved populations to deepen understanding of their needs.
• A more inclusive organizational culture through training and policy development.
We will explore:
• Community-informed service models co-designed with equity-deserving groups to reduce barriers to care.

To deliver on our mission, we will invest in our infrastructure, technology, and processes to ensure operations are efficient, secure, and sustainable—now and into the future.
Strengthen engagement, communications, and advocacy.
We will build meaningful relationships and stand with our clients and community in advocating for better health and social systems. Through inclusive engagement and strategic partnerships, we will amplify voices and drive lasting change.
Rachel’s Story:
A Voice for Our Community
Rachel has been a Vibrant Community Health client for many years, receiving attendant care and healthcare services and participating in our wellness programs. One of her favourite wellness programs is Vibrant’s movie matinees. Rachel is seen by all as warm, thoughtful, and intuitive — always ready to share a kind word or an important insight. A proud university graduate, she enjoys reading and writing, including having authored and self-published three novels.
Rachel values the wide range of supports offered at Vibrant, with the holistic support from both our healthcare and attendant care services, particularly because they are designed intentionally to serve clients with barriers to care. Central to this, is Vibrant’s upholding of the Independent Living Philosophy. To Rachel, the Independent Living Philosophy means having the freedom to make her own choices. This approach to attendant care and healthcare mean that Rachel can lead her own life.
“Some of my friends don’t have the independence that I do. They live with their parents and don’t get to decide for themselves,” she shared. Rachel believes everyone deserves the same right to make their own decisions about their health and life, no matter their abilities. “People think we’re different, but we’re not. We want equal rights and the same choices.”
Advocacy and visible representation matter deeply to Rachel. She uses her voice to inspire change and speak up for a more inclusive, accessible world. Rachel serves on the Vibrant Community Health Advisory Committee along with other clients, where she brings her lived experience and insights to discussions on strategy and activities across the organization. She sees the council as a platform where those who may not always be heard are given a voice. Through her leadership, creativity, and unwavering commitment to inclusion, Rachel exemplifies the strength and spirit of the Vibrant community.
