Today, Vibrant was pleased to welcome Ontario Minister of Health Sylvia Jones, Dr. Jane Philpott, and Associate Minister of Mental Health and Addictions Michelle Cooper to our health centre to celebrate an important milestone in expanding access to primary care across Ontario.
The visit marked a significant achievement in the province's Primary Care Action Plan, with more than 511,000 Ontarians now connected to a family doctor or primary care team. This progress represents an important step toward ensuring that more individuals and families can access the healthcare services they need, when they need them.
At Vibrant, we are proud to contribute to this province-wide effort to strengthen access to equitable, community-based healthcare.
Through the Primary Care Action Plan, 7,231 North Toronto residents have been connected to team-based primary care through Vibrant and our community partners. Earlier this year, working alongside the Sunnybrook Academic Family Health Team and other community primary care sites, we also reached a significant milestone of helping connect more than 10,000 individuals to primary care across North Toronto.
These achievements reflect the importance of collaboration in addressing gaps in primary care access and ensuring that more residents have access to coordinated, comprehensive healthcare services close to home.
At Vibrant, our team-based approach brings together primary care providers, allied health professionals, and community health services to support the overall health and well-being of the people we serve. Today's visit was an opportunity to recognize the collective efforts of healthcare providers, community organizations, and government partners working toward a shared goal of improving access to care.
We extend our sincere thanks to the Province of Ontario and our valued partners at Sunnybrook Health Sciences Centre, the North Toronto Ontario Health Team, Family Health Organizations, and community primary care partners for their continued collaboration and commitment.
Together, we are helping build a more connected, accessible, and equitable primary care system for North Toronto and beyond.
