Nurse Practitioner

Under the strategic direction of the Manager, Health Services, the Nurse Practitioner (NP) functions interdependently with staff to provide shared, client-centered care that emphasizes health promotion, disease prevention and client education. The NP collaborates with physicians, community services and agencies in the provision of primary health care for our clients. The NP practices according to the Standards of Practice of the College of Nurses of Ontario (CNO) and to guidelines and policies established at Vibrant Healthcare Alliance. The NP provides primary health care within the full scope of practice to clients at all stages of their lives.

Salary Range: $116,982.00-$122,178.00 per year Statutory Holidays Required: Rotating on-call basis

Summary of Duties

  • Assuming the primary care provider role in areas of acute and chronic disease prevention and management, consulting with the interprofessional team when necessary.

  • Conducting clients’ interviews and examinations, developing and implementing client-centered treatment plans, while integrating evidence-based health promotion practices.

  • Prescribing medication, including controlled substances, as needed and monitoring for effectiveness

  • Referring clients to other staff or specialists as appropriate.

  • Obtaining, documents and interpreting clients’ information and maintaining clients’ records according to the standard of medical records at Vibrant Community Health including assuring privacy and confidentiality

  • Providing and receiving consults on client care to/from other members of the interdisciplinary team.

  • Contributing to the development, implementation, monitoring and evaluation of medical protocols and directives.

  • Engaging in health promotion programming and activities to support the health of the community.

  • Participating in the clinical team audits and other quality improvements and best practice activities within the organization and broader community health team

  • Maintaining and developing professional competence as defined by CNO through appropriate continuing education opportunities, including accredited activities, self-reflection, and performance review.

  • Ensuring that standards of client care are maintained according to accepted College of Nurses standards, the Regulated Health-Professions Act, and the organization police

  • Advocating on behalf of clients to meet basic health needs and identify advocacy issues to support and inform the board’s efforts to promote healthy public policies

  • Mentoring, teaching, and evaluating students at Vibrant Community Health, as appropriate.

  • Ability to work collaboratively across all levels of the organization

Qualifications/Skills

  • Must be registered in good standing with the College of Nurses of Ontario as a Primary Health Care Nurse Practitioner

  • Masters in nursing from a recognized university or equivalent education and experience

  • Three to five years of recent experience in a primary care role in a community setting or a combination of community experience and relevant hospital or public health nursing

  • Demonstrated ability, commitment to and knowledge of the community health care model including the social determinants of health

  • Proficiency in the use of computers and various software applications, including experience using Electronic Health Record Systems (EHRs)

  • Experience using PS Suite is an asset

  • Excellent interpersonal skills

  • Excellent communication, decision-making, problem-solving, and time management skills

  • Knowledge and experience of primary care issues and challenges for high-risk communities and complex populations, specifically seniors and people with physical disabilities

  • Certification to prescribe controlled substances

  • BLS certification is required

  • Demonstrated ability to work independently and as part of an interprofessional team
