Primary Care Physician

Under the strategic direction of the Manager, Health Services, the Primary Care Physician (Physician) functions interdependently with staff to provide shared, client-centered care that emphasizes health promotion, disease prevention and client education. The Primary Care Physician collaborates with Nurse Practitioners, Allied Health Providers, and community-based services and agencies to provide primary health care for our clients. The Primary Care Physician practices according to the Standards of Practice of the College of Physicians and Surgeons of Ontario (CPSO) and to guidelines and policies established at Vibrant. The Physician provides primary health to clients at all stages of their lives.

In this role, you would contribute to an integrated team-based model focused on creating impactful care pathways and setting new standards in client care. You will be pivotal in fostering an inclusive healthcare setting where equity-deserving people, including people with physical disabilities, older adults and people who use drugs experience the highest quality access to care. Hours of Work: 35 hours weekly FTE Status: 1.0

Weekday Shifts Required: Days, Evenings Weekend Shifts Required: Rotating on-call basis

Salary Range: $256,686-$293,877 Statutory Holidays Required: Rotating on-call basis

Union Status: Non-union Benefit Entitlement: Benefits and HOOPP

Summary of Duties:

  • Assumes the primary care provider role in areas of acute and chronic disease prevention and management, consulting with the interprofessional team when necessary.

  • Performs assessment, diagnosis, counselling, screening, referral, education, treatment and other medical functions as required according to the College of Physicians and Surgeons of Ontario standards of practice.

  • Provides primary care to the general community with an emphasis on Vibrant Community Health’s priority populations

  • Maintains complete and accurate records of client visits including the data required by the Ministry of Health, utilizing the electronic health record

  • Collaborates with other healthcare professionals and health promotion services as necessary and consult as required with Nurse Practitioners to enable them to work within their full scope of practice

  • Ensures complete and up to date documentation of medical records including various performance indicators in accordance with the organization’s funder agreements

  • Contributes to chart documentation and data entry, and participate in chart reviews, case conferences and make referrals

  • Participates in the supervision and education of medical students

  • Continues to improve professional knowledge, develop skills and remain up to date concerning current trends, especially related to Vibrant Community Health priority populations

  • Participates in staff, team and committee meetings, reviews and other activities related to the organization

  • Ensures that laboratory procedural forms are filled out correctly and that specimens are procured and handled properly

  • Works in alignment with the organization’s strategic plan

Qualifications/Skills

  • License to practice medicine and membership in good standing with the College of Physicians and Surgeons of Ontario

  • Medical degree from a recognized university

  • 3 to 5 years’ experience in a primary care setting, community based preferred

  • Experience working with seniors and/or people with physical disabilities an asset

  • Demonstrated ability to work effectively in an interdisciplinary team environment

  • Understanding of the social, environmental and economic determinants of health

  • Knowledge of harm reduction practices and Independent Living philosophy considered an asset

  • Demonstrated excellent interpersonal and effective oral/written communication skills

  • Proficiency in the use of computers and various software applications, including experience using Electronic Health Record Systems (EHRs)

  • Experience using PS Suite is an asset

  • Excellent interpersonal skills necessary to work effectively with our diverse clientele and across all levels of the organization
