Registered Nurse

As a member of an interdisciplinary team, the Registered Nurse functions interdependently with other staff to provide shared, client-centered care that emphasizes health promotion, disease prevention and client education. The Registered Nurse provides support to the primary health care team within the full scope of practice to clients at all stages of their lives. They collaborate with primary care and allied health providers, health promoters and community-based services and agencies in the provision of primary health care for our clients. The Registered Nurse practices according to the Standards of Practice of the College of Nurses of Ontario (CNO) and to guidelines and policies established at Vibrant Community Health.

Summary of Duties:

  • Ensures that standards of client care are maintained according to accepted College of Nurses standards, the Regulated Health Professions Act, and the organization policies

  • Assumes primary care nursing role in the areas of acute and chronic disease prevention and management

  • Monitors clients with episodic and stable chronic conditions commonly seen in primary health care, consults with Physicians and NPs when presented with issues that go beyond the RN scope of practice

  • Performs telephone triage function to clients as part of the primary care team

  • Performs venepuncture, ECGs, and other tests as needed

  • Responsible for upkeep of the clinic lab, including monitoring of inventory and ordering of supplies and equipment as needed

  • Makes internal referrals to other staff as appropriate, and refers clients to external specialists, hospitals or community resources as appropriate

  • Responsible for the orientation of new clients to the organization

  • Assists with the coordination and implementation of quality improvement activities across the clinic

  • Integrates health promotion into interactions with individuals, families or groups through education and counselling

  • Participates in other duties as required

  • Adheres to all policies and procedures of Vibrant Community Health

  • Contributes to chart documentation and data entry, participate in chart reviews and case conferences and makes referrals

  • Conducts client assessments and follow-up visits on site

  • Ensures that the clinical areas and resources, materials and equipment are maintained

  • Facilitate and make presentations as required

Qualifications/Skills

  • Current registration as a Registered Nurse with the College of Nurses of Ontario

  • BSc in Nursing from a recognized university or equivalent education and experience

  • Additional training and/or experience in an area relevant to the organization services

  • 1 to 3 years’ nursing experience in a community setting or combination of community and hospital or public health settings

  • Experience working in community health, with seniors and/or people with physical disabilities an asset

  • Experience in program development, implementation, monitoring and evaluation

  • Maintains knowledge of current social/health issues, and legislation affecting the practice of nursing

  • Comprehensive knowledge of applicable occupational health and safety legislation

  • Demonstrated ability to work effectively in a multi-disciplinary environment

  • Proficiency in the use of computers and various software applications, including experience using Electronic Health Record Systems (EHRs)

  • Experience using PS Suite is an asset

  • Strong oral and written communication skills
