Report an Incident
Use this form to report an injury, safety concern, privacy concern, near miss, or other incident connected to your volunteer role.
Make sure everyone is safe
Get immediate help if anyone is injured or at risk.
Tell your supervisor
Notify the staff member responsible for your shift or program.
Complete the form
Record the facts as soon as possible.
Emergency: Call 911 first if someone is in immediate danger. Complete this form only after the situation is safe.
