Accepting New Healthcare Clients

We are pleased to announce that starting March 17, 2025, Vibrant Community Health’s Primary Healthcare Team will be accepting new clients who meet our eligibility criteria.

We will be accepting clients who currently do not have an accessible healthcare provider according to the following demographics:

• Adults (18+) with physical disabilities residing within the GTA

• Seniors (60+) with 2 or more chronic and/or debilitating medical conditions residing within our catchment area (Bayview to Bathurst and Bloor to 401)

• Clients of one of Vibrant’s Attendant Care Programs

Priority will be given to individuals who:

• Do not have a primary care provider

• Are a member of a priority population described above

• Were recently seen in a hospital or emergency department (within the last 3 months)

• Are socially isolated and have difficulty accessing healthcare

Individuals who meet our eligibility criteria will be assigned to a provider based on provider capacity and must be willing to be assigned to either a physician or a nurse practitioner. The primary healthcare team works collaboratively to ensure that all clients receive the highest level of care. Appointments will be assigned based on priority, and all clients must be able to come in person for appointments if needed.

Vibrant Health Centre is open Monday to Friday and appointments are available between 8:30 am to 4:30 pm, with limited extended availability until 7 pm.
