At Vibrant Community Health, we believe that health is about so much more than just the absence of illness. It is shaped by where we live, how connected we feel, and our daily sense of purpose. That is why we are passionate about social prescribing - a holistic approach to healthcare that is transforming how we support our community.
What is Social Prescribing?
Social prescribing is a simple but powerful idea: not all health needs can be solved with a medical prescription. Think of your health like a giant jigsaw puzzle. Medicine and doctors are incredibly important pieces of that puzzle, but they aren’t the whole picture. Other huge pieces include how much stress you carry, if you have nutritious food, and whether you have friends to talk to. Loneliness isn’t just a sad feeling - it actually affects our physical bodies. In fact, a major health study showed that being alone and disconnected from others can be just as bad for your health as smoking 15 cigarettes a day.
Through social prescribing, our primary and allied care teams can connect you directly to non-clinical, community-based programs and health workshops that support your mental, emotional, and physical well-being. It is about shifting the medical question from “What’s the matter with you?” to “What matters to you?”

Fostering Connection: Groups like our Knitting Group and WeVibe Bingo help community members build lasting friendships and beat isolation
The Power of Song: Our Singalong Group is about much more than just music. Beyond the incredible mental health benefits of singing, it acts as a gentle, evidence-based workout for your lungs. For folks living with COPD (Chronic Obstructive Pulmonary Disease) or other respiratory conditions, singing helps improve diaphragmatic breathing, lung capacity, and breath control in a joyful, supportive space
Active Living & Fall Prevention: Alongside classes like Fit for Life, Gentle Exercise, and Adaptive Chair Yoga, we are proud to offer Strong & Steady - our specialized, evidence-based fall prevention program. Designed to help older adults maintain their independence, this program focuses on building functional strength, improving balance, and boosting confidence in everyday movements
Peer Support & Community: Our People on Wheels (POW) group provides an invaluable peer space for those living with Multiple Sclerosis, while our Newcomer Support Group helps Torontonians find their footing in a brand-new city
Creative & Mindful Outlets: Programs like the Adult Colouring Program or our Mindfulness Drop-In offer a quiet, relaxing space to reduce stress and discover a sense of inner calm

Talk to Your Provider: During a regular appointment, let your provider know if you’ve been feeling lonely, stressed, inactive, or concerned about your balance, breathing, or nutrition
Get Connected: They can write a “social prescription” and connect you directly to our Health Promotion and Clinical Education teams
Find Your Fit: We will help you find the exact right program(s) that matches your interests, comfort level, and health needs
Anyone can join! You don’t need a formal prescription to get involved. Our health promotion and wellness programs are completely free and open to community members.
