Welcome Message from the Leadership

This past year has been a period of transformation and renewal for Vibrant Community Health. Together with our staff, volunteers, partners, and community, we have navigated significant milestones that will shape how we continue to provide care and connection in North Toronto.

One of the most important changes was our new name: Vibrant Community Health. This shift reflects not only who we are today but also who we aspire to be — a welcoming, inclusive, and community-driven health centre where everyone feels they belong. Also, 2024-25 was a pivotal year, with the exploration, client consultation and fully board of directors and membership move forward with the governance restructuring integration with Sunnybrook Health Sciences Centre.

Our commitment to accessibility and connection extended online with the launch of our new website, designed to make it easier for clients to access information, book virtual appointments, and learn about our programs. We also made important site improvements to ensure our spaces remain safe, welcoming, and accessible for all. None of this would be possible without the dedication of our team.

Looking ahead, our priorities remain clear: to strengthen primary care, to expand wellness and outreach programs, and to support priority populations who face barriers to health equity. We will continue to listen, adapt, and innovate — always guided by the voices of the community we serve.
